Payment and Fees
Bills will be mailed to all patients at the end of the month.
If you plan to use insurance, and have out of network coverage for which you will submit a claim, it is important to understand the possible implications of doing so. To obtain coverage for psychotherapy services, I am required to provide your insurance company with a diagnosis from the DSM-V classification of mental disorders from the American Psychiatric Association. In addition, the insurance company will have the right to limit the number of sessions, and ask for results of testing and a list of your symptoms. This will become part of your permanent health record. As more health information is communicated and stored electronically, there may also be concern about the safety and use of your health information. You can visit www.MIB.com and click on Consumers for some examples of where your personal health information may be stored and used. This is not an attempt to discourage you from utilizing your insurance, but intended to help you make an informed decision.
It is also important to know that most insurance companies do not provide coverage for brief clinical hypnosis.

Please check your coverage carefully by asking the following questions, being sure to clarify out-of-network coverage:
* Do I have outpatient mental health/behavioral health benefits? You should check under CT State Law.
* What is my deductible for the year and has it been met?
* How many sessions per year does my health insurance cover?
* What is the coverage amount per therapy session?
* Is a referral required from my primary care physician?
Cash and check, made payable to Yvonne B. Bregman, currently accepted for payment at the beginning of each session. Session fee is $175.

You are responsible for full payment at each session. I will provide you with a monthly receipt which you can submit with your claim form and which will contain all the information your insurance company requires to process your claims.
Sliding scale services are available on a limited basis with proof of need via W2, tax returns, etc.
If you do not keep your scheduled therapy appointment, and you have not notified me at least 24 hours in advance, you will be required to pay the full cost of the session.
